Licensed Practical Nurse (LPN) - Per diem - All Shifts - UCP of Long Island

Our two seven bed children's residences in Central Islip , a seven bed medically fragile residence, also in Central Islip as well as our 31 bed medically fragile facility in Commack are in need of per diem LPN's to assist on all shift, morning, evening and overnight.

Rate is $28.50 per hour plus shift differentials (5%-10% depending on shift) for evenings, overnights and weekends.

Full Time and Part-time positions also available.

Licensed Practical Nurse (LPN)

Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served.

Job Responsibilities:

• Administer medications and treatments as prescribed by the physician
• Generate incident, seizure, and illness reports
• Monitor and report health care on daily flow report records
• Instruct direct support professionals in the above, i.e., hygiene, diet, weights, etc.
• Submit written monthly, quarterly, annual nursing/medical progress notes

Required education: Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.

Required license or certification: New York State Licensed Practical Nurse
