Title: Licensed Practical Nurse (LPN)
We’re looking for a Licensed Practical Nurse (LPN) to join our team at our residential program based in Burlington MA
Job Summary: The LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence. The LPN will follow established standards of care and operate within their scope of practice as defined by Massachusetts Nurse Practice Act except where limited by Northeast Arc’s policy and procedure.
Pay rate: $34 hourly.
Schedule: 9am-3pm or 8am-2pm (30 hours per week) Monday through Friday
Primary Responsibilities:
- Understand the clinical and social needs of the individuals' receiving services.
- Serve as a liaison between the community and the home.
- Administer medications as prescribed and ensure safe and proper storage of medications as required by MAP and the Northeast Arc. Complete narcotic count with MAP-certified and/or other licensed personnel at change of shift.
- Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care.
- Integral part of the team working closely with the Program Director, RN Case Manager, and direct support staff.
- Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed.
- Present information regarding any individual in a professional manner in all settings including: meetings, phone contact, daily written documentation and /or email correspondences.
- Responsible to assist at physician's appointments, community trips, and other activities as assigned.
- Maintain confidentiality of any information relating to individuals, their families and activities within the home.
- Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.
- Communicate all health-related matters to RN Case Manager, Program Director, on-call RN, and/or designee.
- Maintain a license in good standing and certifications as required by the program, the funding source, and OSHA including but not limited to provider-level CPR, Universal Precautions, and Human Rights.
Requirements:
- Licensed LPN in MA with minimum 1 year experience
- Experience with multiply handicapped/developmentally disabled individuals a plus
- Must have a valid Driver’s license and proof of a good driving record
