Hours: Full-time | Days | 8:00am - 5:00pm
Territory: This role involves visiting patients in their homes, primarily in the Southwest Twin Cities Metro area, with the office located in Roseville, MN.

Requirements:
- Current LPN license in MN, active and in good standing.
- 2 years of LPN experience.
- Valid driver's license, reliable transportation, and proof of auto insurance.
- CPR Certification.
Key Responsibilities:
- Participate in ongoing patient assessments.
- Collaborate on care plans with patients and caregivers.
- Administer physician-ordered services as per care plans.
- Report changes in patient conditions to the RN.
- Educate patients and families on healthcare needs.
- Document clinical notes accurately and timely.
