Grand Victorian of Rockford is seeking a Licensed Practical Nurse (LPN) to join our team!
Responsibilities:
- Administers medications as stated on physicianorders, ensuring strict adherence to Community medication managementdocumentation and procedure, and demonstrating steadfast dedication to residentsafety
- Upon thorough assessment, creates personalizedservice plans tailored to individual care needs while championing residentindependence and dignity
- Cultivates communication between physicians,healthcare providers, and family, fostering effective communication pertaining to the physical, mental, and social well-being of ourresidents
- Contributes toward quality assurance andperformance improvement initiatives
- Engages in continuous learning and developmentopportunities through attendance and participation in educational seminars,staff meetings, and other learning experience
Qualifications:
- Current state licensure as Licensed PracticalNurse (LPN)
- Current CPR certification
- Exceptional communication and interpersonalskills
- Must demonstrate the ability to serve with love,compassion, and dignity
