JOB SUMMARY
This position encompasses a variety of duties essential in providing case management, supervising and performing direct and indirect care to residents in accordance with resident care plans and physician orders and maintaining a safe and healthful environment.
PRIMARY ACCOUNTABILITY
A thorough knowledge of the practices and procedures in the use and function of Licensed Practical Nurse principles. Strong written and oral communication skills. Effective in establishing and maintaining rapport with staff, residents, resident's families, and visiting doctors. Must possess the ability to maintain composure under pressure. A professional appearance is to be maintained at all times. The Licensed Practical Nurse will possess a genuine interest in and concern for geriatric, chronically ill, and/or disabled persons. Must be able to fluently communicate in or to take or give direction or guidance in the English language.
ESSENTIAL FUNCTIONS
- Regular attendance is considered an essential function of this position and is necessary for the efficient operation of the business.
- Serves as an ambassador of goodwill to all potential and current residents and staff.
- Participates in establishing, implementing and updating individualized plan of care for each resident.
- Administers medications and treatments to resident in accordance with physician orders; accurately and promptly documents administration of medications and treatments; observes and evaluates effectiveness of medications and treatments in accordance with facility policies and procedures.
- Maintains accurate and timely documentation in medical record of observations and care delivered to each resident in accordance with facility policies and procedures.
- Responsible for documenting changes in condition of resident and reporting to physician, nursing supervisor, unit coordinator, and family in timely manner in accordance with facility policies and procedures.
- Participates in team conferences and staff meetings.
- Responsible to receive report from out-going nurse and give accurate report to on-coming nurse regarding status of each assigned resident.
- Provides direction to, supervising, and monitoring of Certified Nursing Assistants.
- Monitors and maintains adequate supply of medications, biologicals, and medical supplies necessary to meet the needs of the resident; stores and distributes same in safe and sanitary manner observing facility policies and procedures; responsible to allocate charges for same to appropriate resident or account.
- Monitors availability, cleanliness and safety of equipment used by resident.
- Assumes charge nurse duties when designated, including supervision of overall unit operation.
- Practices Universal Precautions and infection control measures and observes safety precautions and protocols in the performance of all duties and at all times while on facility premises.
- Demonstrates courteous and respectful verbal and physical interactions with residents, physicians, family members of resident, visitors, and other staff.
- Demonstrates respect for resident by maintaining resident's dignity and confidentiality, providing privacy and abiding by the resident's Bill of Rights.
- Responsible for attending in-services and staff meetings, as required by facility policies and procedures.
- Takes direction from and reports to charge nurse, House Supervisor, Assistant Director of Nursing, or Director of Nursing; performs all duties of Licensed Practical Nurse competently and with a positive and cooperative attitude.
- Performs all duties normally assigned to a Certified Nursing Assistant, if required.
- Responsible for answering, in an expedient manner, all call lights within sight and hearing.
- Greets all visitors in a friendly, courteous and professional manner, provides information and guidance.
- Performs other duties as may be assigned.
