Responsibilities:

• Provide skilled nursing care to patients in their homes under the direction of a registered nurse or physician.
• Educate patients and their families on proper care techniques and medication management.
• Collaborate with interdisciplinary team members to ensure the highest quality of care.
• Perform assessments, administer medications, and provide wound care as needed.

Qualifications:

• Graduation from an accredited LPN program with a valid LPN license in Pennsylvania.
• Previous experience in home health or related setting preferred but not required.
• Strong clinical skills and ability to work independently.
• Excellent communication and interpersonal abilities.
