The Nurse Care Manager supports a caseload of approximately 225 clients with severe and persistent mental illness with their behavioral and physical health needs. The Nurse Care Manager champions a holistic, person-centered approach and is responsible for providing training on medical diseases, treatments, and medications, both individually and in a group setting. The Nurse Care Manager assists in identify gaps in care and developing recovery plan goals for clients with co-occurring chronic disease. Using a team approach, the Nurse Care Manager will consult with IHSs and other team members regarding identified health conditions and provide care coordination and interventions as needed. Nurse Care Manager is responsible for contacting medical providers and hospitals regarding hospital admission and discharge. Duties may also include assisting in the clinic, in home visits, assisting with medication boxes, and administering injections. This position may require some work in th
e community. Hours of employment are Monday through Friday, 8:30 AM to 5:00 PM. This position may require some work in the community.
ESSENTIAL FUNCTIONS JOB SPECIFIC
Champions healthy lifestyle changes, including tobacco cessation and prevention, nutrition, physical activity and substance use prevention.
Coordinates care with external healthcare providers.
Receives, identifies and follows-up treatment and medication alerts reported by HIT tools.
Consults with CSSs about identified health conditions of their clients and provides educational training on chronic disease states, health coaching, medications and healthy living.
Coordinates with the CSSs regarding client hospital admission, course, and discharge; including medication reconciliation and identification of post discharge concerns. Communicates medications concerns to appropriate provider.
Conducts or reviews annual health screening with the client and identifies health and wellness needs/goals for the clients recovery plan.
Maintains documentation of HCH activities in client charts and any collaboration of HCH team meetings pertaining to client care.
Participates in team staffings for high acuity clients with the Primary Care Physician Consultant and treatment team.
Participates in monthly HCH Team meetings with the Physician Consultant, HCH Director, and others as appropriate, and attends state required meetings and trainings
Takes and records vitals and performs Metabolic Screenings.
Administers injections, as needed.
Assists in clinic, as needed.

The employee is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.

Experience: Minimum of two years of medical nursing experience, in a primary practice location, preferred. Experience in behavioral health, substance abuse prevention and education helpful.
Licensure, Certification and/or Registration: Current licensure as an LPN in the State of Missouri.
Skills required: Ability to work within a multi-disciplinary team, excellent written and verbal communication skills, flexibility and ability to multi-task.
