About the Job
Duties and Responsibilities:
- Responsible for completing a face-to-face initial/ongoing Comprehensive Assessment (CA) with consumers.
- Interview consumers to obtain information concerning their medical history and other pertinent information.
- Assess the counseling needs of the consumers.
- Assess consumers using the ASI or other standardized clinical interview tool and diagnose consumers for substance use disorder using the DSM-5.
- Determine the appropriate level of care according to ASAM Criteria
- Obtain previous records that are relevant to the current treatment episode unless clinically contraindicated, and collaborate with the consumer to develop a written treatment plan that includes goals and measurable objectives, and is consumer-centered, and recovery-oriented.
- Provide the substance abuse counseling services specified in the consumer treatment plan.
- Review consumers throughout the treatment episode according to the ASAM Criteria, to determine the need for continued services, transfer, or discharge/transfer.
- Review and, where necessary, revise the consumer’s treatment plan to address ongoing problems.
- Develop a consumer discharge/transfer plan to ensure movement to the appropriate levels of care.
- Participate as a member of a multidisciplinary team for assigned consumers.
- Provide active case consultation.
- Document all counseling and education services, assessments, reassessments, referrals, and follow-ups in the consumer's clinical record, and provide appropriate signatures and dating of such entries, including those made in electronic records.
- Retains the responsibility of meeting face-to-face with the staff to review the assessment and treatment plan of the consumer for the development of an action plan to meet the consumer’s needs.
- Progress notes are due daily.
- Report directly to the Chief Executive Officer (CEO).
Minimum Qualifications:
- Must at least possess a master’s degree in a behavioral health-related field including, but not limited to, psychology, nursing, counseling, or social work with addiction treatment experience. Must also possess board certification and licensure in the state of New Jersey for one of the following: Licensed Clinical Social Workers (LCSWs), Licensed Social Workers (LSWs), Licensed Professional Counselor (LPC), or Licensed Clinical Alcohol Drug Counselors (LCADCs).
- Submission to a criminal background check and be determined to be not disqualified by the Department of Human Services prior to working with individuals served.
- Agreement to have one’s name screened against the Central Registry of Offenders Against Individuals with Developmental Disabilities.
- Drug Screen are required initially, at random, and for a cause.
- All employees shall submit to drug testing initial, random, and for cause.
- Submission of a completed application to have your name checked against the Child Abuse Registry Information (CARI).
Required Training and/or Certifications.
- Successful completion and demonstration of proficiency in all areas of required training.
- CPR, First Aid, Emergency Procedures, Medication Administration (if applicable).
- Supervisory experience is a plus.
