Position Summary
Under general supervision, the LCSW provides comprehensive case management services for all clients and their family members. Responsibilities include initial and on-going assessment of bio-psychosocial factors, healthcare factors and educations needs of these clients with a focus on enhancing our client's capacity for self-support and self-sufficiency.

Essential Duties and Responsibilities
Direct Patient Care - responsible for providing quality direct primary care to patients and family members.
- Provide crisis intervention services to patients
- Provides/ conducts psychosocial assessments and assists the patient in utilizing the support systems and community resources available.
- Provide patient education
- Provide counseling therapy
- Implement treatment plans by identifying an ranking problems based on individual patient needs.
- Support the medical treatment plan.
- Demonstrates critical thinking skills by performing appropriate interventions.
- Identifies the impact of cultural diversity and lifestyle choices on patient condition and makes accommodations for such differences.
- Meet with individuals to discuss current needs and create plans to address needs.
- Utilizes knowledge and required skills to ensure appropriate assessments.
- Consult and plan meetings with other professional to discuss patient's treatment services.
- Prepare and maintain records.
- Maintain and up-to-date referral system.
- Contributes to optimal efficiency and productivity of the clinical care team by anticipating needs of patient and communicating with medical provider regarding patient care.
- Participation and leadership in all center based activities Coach patients to engage in various wellness programs offered by center, based on care plan.
- Meets productivity and billing goals set by organization.
- Generates reports based on work with clients.
- Work with the Clinical Services Department to promote ongoing performance improvement activities.
- Perform other duties as assigned or become necessary by supervisor
Supervisor Responsibility
- None
- Masters of Social Work (MSW) or Master’s Degree in Human Services field.
- Must have an active NC LSCW license.
- Minimum of 4 years of professional experience working as LCSW in a community healthcare environment.
- Knowledge of federal, state, and local laws/regulations pertaining to our program services.
