As a Lead Counselor, you will assist with coordination of all treatment aspects of a patient’s care. This role serves as a professional teammate and/or leader of an interdisciplinary team and is responsible directly to the Clinical Director for the coordination and continuity of care from admission through discharge and follow-up of the patients. The Lead Counselor will also assist the Clinical Director in problem solving, conflict resolution and readiness for state CARF audits.
Requirements:
• Bachelor’s degree specializing in psychology and/or counseling from an accredited
college or university.
• Three (3) years’ experience in substance abuse and/or related field.
• Must obtain one of the following Licensure or certification for counseling:
o Licensed Chemical Dependency Counselor III (LCDC III), Licensed Independent
o Chemical Dependency Counselor (LICDC), Licensed Independent Chemical
o Dependency Counselor-Clinical Supervisor (LICDC-CS), Licensed Independent
o Social Worker (LISW), Licensed Professional Clinical Counselor (LPCC),
o Psychologist, Psychiatrist, Independent Marriage, and Family Therapist (IMFT)
Preferred education and experience:
• Master's degree in psychology, Counseling, Social Work, or a related field
• Five (5) years of experience in counseling or case management
Responsibilities:
- Ensures completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation as required by state, federal, and accreditation standards.
- Maintains a caseload up to twelve (12) patients.
- Performs all tasks related to counseling to include completion of master treatment plan, individual service plans, ARTS, biopsychosocial, discharge summaries and
individualized/group therapy. - Assures that patients are assessed and placed in appropriate phases of treatment according to state and federal guidelines.
- Counsel patients and address specific needs of the people served.
- Arranges referrals and community information to patients as appropriate.
- Assures all documentation is collected from patients as required by state and federal standards, within the regulated timeframes.
- Submit required statistical and other data as required.
- Shall provide and obtain the following therapeutic services as determined by the Treatment Team and specified in the patient’s Individual Treatment Plan: individual counseling, family counseling, group counseling, and community activities.
- Report all relevant developments and information affecting the patients and facility operation.
- Shall provide teammate coverage as scheduled.
- Complete administrative tasks
- May conduct duties of the Clinical Director in interim capacity when Director is off-site.
- Coordinates care with the multi-disciplinary team, including but not limited to developing a system to ensure UM requirements are met with Clinical Coordinator or Clinical Director
- Enliven compassionate care and empower patients to realize their best level of functioning in the community.
- Attend team meetings and complete all training courses timely as required.
- Other duties as assigned.
