Lead Care Coordination Specialist - AIDS Foundation Chicago

The Lead Care Coordination Specialist is responsible for determining eligibility of clients with short term needs for Ryan White services. The Lead Specialist serves clients at the AIDS Foundation Chicago as well as at subcontractor agencies based in Cook and the Collar Counties. The first and highest priority of the Specialist is to ensure that persons living with HIV/AIDS are linked to appropriate care and provided services and referrals to stay engaged in care.. The Lead Care Coordination Specialist will manage a client case load as well as assist the Ryan White Program Manager in managing client databases ensuring eligible clients are linked and retained in care. In the lead role, the Lead Care Coordination Specialist also assists and guides other Care Coordination Specialists.

The salary range for this role is $52,000 to $53,500 annually. 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Assessments & Care Plans

• Conduct face-to-face and over the phone assessment and reassessments every six months to assess client needs

• Create care plans that match the identified needs of the client

• Ensure that all clients referred to services are eligible prior to the date of service

• Support the Senior Program Manager in answering assessment – and care plan-related questions and providing guidance to the Care Coordination Specialist as needed

Client Service Coordination and Guidance

• Refer clients to appropriate services highlighted in the care plan

• Refer clients to more intensive case management services if needed

• Refer clients to other core and supportive services if determined to be necessary

• Complete emergency housing applications for eligible clients

• Complete Medication Assistance Program (MAP) applications

• Provide insurance benefits navigation as needed

• Travel to meet clients at home, clinical, or community-based setting as needed

• Provide technical assistance to case managers regarding the client database

• Compile a list of new agencies that serve persons living with HIV/AIDS and conduct outreach

• Distribute food vouchers and transportation cards according to eligibility and need

• Guide and support the work of assigned Care Coordination Specialists

• Schedule and assign client service coordination activities

Client Data Tracking and Administrative Support

• Document encounters using designated client database

• Document need for Emergency Financial Assistance and Medical Transportation

• Identify community meeting spaces to have client meetings that are conducive to maintaining confidentiality

• Collaborate with subcontractor agencies to schedule on-site assessments/re-assessments

• Schedule client appointments both at AFC and subcontractor agencies to complete certifications

• Perform record-keeping and clerical functions (e.g., scheduling, copying, faxing, data inputting, transmitting, telephoning, taking messages, responding to written and verbal inquires, etc.) for the purpose of supporting the needs of the client

• Monitor and track activities completed by care coordination specialist and implement quality improvement initiatives as needed

• Schedule and lead regular internal care coordination team meetings

• Schedule and lead regular external care coordination team meetings with partner agencies

Other

• Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others

• Complete at least 12 hours designated on-going training annually

• Assist the Senior Program Manager with special care coordination-related projects

• Protect organization's value and manage risk by keeping information confidential

• Perform other duties as assigned

EXPERIENCE AND EDUCATION

Minimum Qualifications

• Associates degree in a human services field OR at least two years of experience in Social Services, Health Care or related field

• Basic knowledge of HIV/AIDS prevention and treatment

Preferred Qualifications

• Bachelor's degree in the Human Services field (i.e., Psychology, Sociology, Public Health, Social Work)

• Bilingual, Spanish-English

WORK ENVIRONMENT AND PHYSICAL DEMANDS

The work environment is representative of that found in a general office setting. Tasks involve the ability to exert light physical effort in sedentary to light work, but may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (20-25 pounds). Tasks may involve extended periods of time at a keyboard or workstation. Minor community travel may be required.
