POSITION DESCRIPTION:
The Lead Advance Practice Professional (APP) of Trinity Health IHA Medical Group, Neurology, is charged with leading the advanced practice team at Trinity Hospital – Ann Arbor, Trinity Hospital – Oakland Service. The Lead APP will report to IHA Neurosciences Service line chief. In addition, the person will coordinate with the Neurology Service Line Program Manager, the Practice Managers of each location, and each Hospital leadership to guide the APP service in providing quality patient care and service to both internal and external customers.
ESSENTIAL JOB FUNCTIONS:
APP Responsibilities
Performs comprehensive and interval history and physical exams of patients, identifies preventive health measures and/or establishes priorities of care for the presenting problem within prescribed guidelines and instructions of supervising Physician.Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.Records physical findings and formulates plan and prognosis, based on patient's condition; documents patient visit appropriately in the electronic medical record (EMR).Assists in the development of a treatment plan and appropriate follow-up; discusses case with Physician and other health professionals to prepare comprehensive patient care plan.Educates patient regarding the diagnosis; reviews treatment plan.Submits health care plan and goals of individual patients for periodic review and evaluation by Physician. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to Physician for consultation or to specialized health resources for treatment.Keeps current on new knowledge gained from conferences, workshops, professional literature, or other training and assimilates this knowledge into clinical practice.Lead APP Responsibilities
The Lead APP will support Trinity Health IHA Medical Group, the Hospitals and the Division of Medical Specialties-Neurology in the following:
Promotes professional growth, development and successful performance of Advanced Practice Professionals-Neurology.Actively supports recruitment and orientation of new APPs.Provides oversight for the timely and equitable completion and distribution of APP work schedules.Attends monthly team leadership meetings and monthly night provider meetings. The Lead works with the Practice Managers to coordinate quarterly APP meetings.Provides guidance, mentoring and coaching in conjunction with the essential job functions as identified in the IHA Neurology APP position description.Collaboratively designs, implements and monitors clinical processes and helps build clinical teams with clear roles and expectations that support the delivery of outstanding patient care.Supports the comprehensive stroke program by supporting delivery of evidence-based acute stroke assessment and management, including consultation and practice oversight; collaborates with program leadership to develop and disseminate acute stroke continuing education programs; participates in performance improvement processes; participates in comprehensive stroke center research.Acts as liaison between hospitalist APP group, hospital leadership and Trinity Health IHA Medical Group leadership.Demonstrates the value of IHA Neurology APP program to the hospitals and Trinity Health IHA Medical Group leadership.Directly communicates effectively and in a timely manner with neurology APPs to continually update, engage and educate the hospitalist APP team.Manages program operations in coordination with Chief of Neurosciences, Associate Director of MS Neurology and Practice Managers of Trinity Health IHA Medical Group Neurology to build APP schedules to ensure service is fully staffed.Supervises the overall activities of the Advance Practice Professionals on all inpatient internal medicine services.Conducts biannual provider performance evaluations, periodic coaching sessions, orientation programs and improvement/teaching plans designed to maximize providers’ potential.Conducts disciplinary actions and attendance review; provides feedback to APP staff.In collaboration with the Practice Manager, coordinates APP student schedules.Reviews patient complaints and adverse event reports (related to APPs) generated through reporting system, performs chart reviews when asked by DH, and shares with providers and IHA hospitalist leadership.Meets regularly with leadership of STAT RN/RRT meeting.Meets regularly with leadership of the MSJ Service.Assumes additional duties as required.Other Responsibilities

Advanced Practice Nurse (APN)
Master’s degree in NursingState of Michigan Nurse Practitioner licenseCertified through the National board depending on specialtyState of Michigan license as a RN and specialty certification as a Nurse Practitioner Physician’s Assistant
Bachelor’s degreeCertified though the National Commission on Certification of Physician Assistants (NCCPA)- State of Michigan license as a Physician’s Assistant

Ability to effectively supervise and manage functional area within the practice, providing feedback and guidance to staff.Demonstrated competency in obtaining accurate patient vital statistics (height, weight, blood pressure, temperature, etc.) and patient health history.Proficient in medical terminology.Proficiency in operating a standard desktop and Windows-based computer system, including but not limited to, electronic medical records/templates, NextGen, Microsoft Office, Outlook, email, e-learning, intranet and computer navigation. This job operates in a medical facility and requires regular walking to various locations around the hospital/clinic. Employees will be working where there is patient care equipment. Hazardous materials, including blood borne pathogens and bodily fluids are also present. Exposure to sharps, x-rays, patients' conditions and some unpleasant sights, smells and contagious diseases are possible. This position requires significant interaction with people (many of whom are scared, hurt and/or ill) which can be stressful and result in competing priorities.
