Description:
In need of a LCSW with discharge experience required
*** This person will need to be independent problem solver, clinical social worker would be preferred, and must have complex discharge experience. Must work well with the formal discharge planner on site and providing post care planning with the patient.
Start date: ASAP
Shift: 5x8HR shifts (M-F 08:00-16:30)
Length: 13 or 26 week options with possible extension
Years of experience REQ: 1 year, will accept new grads with clinical social work experience
First-timers accepted: Yes, willing to review with the right experience
Weekend REQ: possibly
Certs REQ: MSW and BLS, LCSW preferred
Will this traveler need to float between like units: yes, floating as needed between discharge locations.
Locals accepted: Not at this time. What is the local radius rule? 50 miles
Pending License accepted: No, WA license must be in hand at apply
RTO Restrictions: none, please submit all requests at apply. Must work 2 of the 3 winter holidays.
Unit notes: This is a new program in the state of WA. This position will be part of the complex discharge pilot / enhanced care management. Preferred Experience in guardianship, working with Client, brokering with outpatient, housing, homelessness.
Special Requirements: **local travel is required, may need to go between Colby Campus/ Pacific Campus/ other local shelters for patient care and follow up.
