Internal Medicine Physician Assistant - Tandym Health

A health system in Columbus OH, is actively seeking a dedicated Nurse Practitioner to join their team. In this role, you will lead the medical aspects of team-based care for individuals with serious mental illness, conducting whole-person assessments while collaborating closely with community primary care and behavioral health providers.

Responsibilities:

• Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities.
• Support care coordination.
• Lead care and disease management, addressing care gaps.
• Coordinate with care providers at CMHCs, FQHCs, and hospitals.
• Provide direct patient care for unscheduled needs.
• Perform other duties as needed.

Qualifications:

• OH Nurse Practitioner (NP) and/or Physician Assistant (PA) license

Desired Skills:

• 2+ years of experience as a Nurse Practitioner or Physician Assistant
• Experience with behavioral health patients
