InTake/Scheduling Coordinator - Vicdania Home Health

The Intake/Scheduling Coordinator will:

• Provide superior customer service while answering the phones and directing customer calls to the appropriate staff.
• Monitor and address referrals from different online home health referral portals.
• Verify insurance eligibility for incoming referrals and refer to clinical team for review.
• Determine home health disciplines needed from referral and verify that patients are established with PCP.
• Answer phones, receive all intake information and enter referral information into EMR
• Complete intake forms and route them appropriately for admissions approval to Clinical Director
• Initiate authorization process, and review of cases on a weekly basis
• Schedule initial discipline visits (SN, PT, HHA).
• Call potential patient on the day before admission to verify discharge date and inform them that agency clinician will call to schedule initial visit.
• Communicate on a routine basis with all Intake partners to maintain smooth relations and to obtain needed information
• Confer with Clinical Director on all complaints; ensures that appropriate documents are compiled and a response is provided to appropriate stakeholders in a timely manner
• Must have substantive experience in working referral/Intake related tasks in an interdisciplinary (RN, PT, OT, ST, CSW, CNA) Home Health Agency or related Health Care Organization
• Complete mandatory Missed Visit Reporting as directed by the state.
• Create, maintain and handle staffing/client scheduling.

Qualifications for an Intake/Scheduling Coordinator:

• High School Diploma and preferrable Medical Coding/Billing education.
• Prior knowledge of/experience with Medicare, Medicaid and commercial insurance prior authorization process.
• Prior medical office experience preferred.
• Strong computer skills required (electronic medical record).
• Excellent communication and interpersonal skills.
