- Work Shift: Monday - Friday, 9AM-5:30PM (Friday work from home in 2025)
Intake Manager
The Intake Manager will manage the referral process and the intake team for all sites. They will be serving as a liaison between the hospital, health plan or other partnering agency and the NHF Recuperative care staff, ensuring a smooth transition from hospital to Recuperative Care Program facilities for our Los Angeles and Ventura County facilities and any other facilities we operate that may need assistance. Assures information is gathered from hospitals and referral partners accurately and thoroughly according to established program policies and procedures. They will be tasked at fostering a positive and collaborative partnership with our partners, providing quality customer service and clear and effective communication.
Responsibilities and Initiatives
To help NHF meet its growth goals, the Intake Manager will:
- Serve as a point of contact for partners on referrals, providing internal and external updates on referral status. Check and maintain count of bed reservations, address deficiency where applicable.
- Oversee scheduling and management of intake team including team and 1-1 meetings.
- Ensure accuracy of the intake team with proper completion of the referrals received. Review to ensure referrals are entered in a timely manner into a database, confirming partners have received a response within a short time.
- Create and initiate guest files in both NHF CRM database. Maintain confidentiality of patient information.
- Communicate effectively with team about referrals and admissions.
- Build trusting relationships with Staff, partners and guests. Deliver excellent customer service and exercise good judgment with a compassionate, positive attitude while managing a population of guests who may be experiencing difficult life events, such as homelessness, unemployment, addiction recovery, health issues and/or poverty.
- Will gather statistics and data about intakes and work with Director of Account Relationships to improve program based on this data.
- Ensure accuracy of billing data in collaboration with intake team and Director of Account Relationships.
- Provide input and evaluation of programming for quality improvement purposes.
- Work collaboratively with other members of the team, including active participation in department and agency staff meetings and training. Assist in providing information and support to the team.
- Flexibility to adapt to schedule changes to meet the needs of the facility.
- Must be able to aid other NHF facilities as assigned by supervisor when the need arises.
- Complete additional tasks as assigned to meet the needs of the facility, notwithstanding primary roles.
- Always follow NHF policies and procedures.
Requirements
- 2-3 years’ experience managing a team.
- 2-3 years’ experience of working in intake or similar in a health care or non-profit setting preferred.
- Proficient in Microsoft Operating system and database applications; knowledge of CRM software, and website content management systems a plus, but not required.
- Ability to multi-task.
- Detail-oriented with exceptional time management and organizational, written, verbal, interpersonal and computer skills.
- Ability to learn quickly and work effectively with minimal supervision.
- Reliable transportation.
- Bi-lingual in Spanish. (Preferred, not required).
- May require TB test for some locations.
- May require Fingerprint live scan/background.
Physical Requirements
Work is performed in an office environment and requires the ability to operate standard office equipment such as a computer, keyboard, and telephones. Work is performed for an extended period behind a computer. You will need to be able to talk or hear, in-person, over the phone, or via telecommunication without assistance. On occasion will have multiple lines ringing at the same time. Very limited traveling will occur for the position.
