In this role as a Staff Pharmacist, you will provide a quality level of comprehensive pharmaceutical services demonstrating sound judgment based upon knowledge and application of professional, departmental and organizational principles and standards.
The person in this role demonstrates the necessary skills and knowledge to provide care to the patient population served by the organization which includes neonate, pediatric, adolescent, adult and geriatric patients. The Clinical Staff Pharmacist is responsible for providing pharmaceutical care consistent with departmental/hospital philosophy, mission statement, professional standards, and policies and procedures, in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Observes and adheres to all corporate compliance requirements and communicates issues as they arise to the appropriate parties as identified in the Corporate Compliance Plan.
Essential Job Functions
- Demonstrates a commitment to enhancing the services of the department through personal actions and/or participation on departmental ad hoc performance improvement teams.
- Reports cost saving (expense reduction) possibilities to pharmacy supervisory personnel for immediate consideration.
- Monitors inventory usage of all drugs and pharmaceutical items throughout the pharmacy and requests replenishments as required.
- Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection, and suggesting appropriate Formulary alternatives to the physician as indicated
- Insures that technicians follow proper aseptic technique in preparing IV admixtures
- Utilizes Dose Edge IV Workflow processes for preparation and check of completed admixture for accuracy and clarity.
- Assures accurate entry of hyper alimentation formulation into CAPS computer software program
- Oversees preparation of hyper alimentation solutions as needed
- Oversees chemotherapy medications preparation and adherence to hazardous material waste guidelines.
- Monitors drug therapy for efficacious and adverse effects and notifies the physician when changes are necessary.
- Recommends medications to physicians when appropriate.
- Responds and participates in all Code 33's, as assigned, by preparing all medications, monitoring drug therapy and performing CPR if necessary certification.
- Responds and participates in all Code 66’s, as assigned, by preparing tPA, collaborating with interdisciplinary team members, and ensuring appropriate documentation per institution guidelines.
- Detects and follows up on all reports of suspected drug allergic and adverse reactions and documents as required.
- Initiates established protocols for pharmacokinetic services by evaluating patient, drug, and dosage; then, ordering appropriate follow-up work as requested by physicians and per departmental guidelines (protocols).
- Interviews patients as necessary to determine past medical history and previous medications, present status, and response to therapy
- Interprets laboratory, radiology and other patient-specific data to aid in determination of treatment plans and monitoring of response to therapy on an on-going basis, makes dosage adjustments as appropriate according to Pharmacy and Therapeutics Committee approved Renal Dosing Guidelines and IV to oral conversions.
- Utilizes pharmacokinetic principles in formulation of therapeutic drug regimens per department protocol.
- Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, religious/spiritual needs of patients and their families.
Required Qualifications
- The incumbent must remain alert on all occasions, and must be able to effectively intervene if questions arise as to the appropriateness of a medication order
- The ability to significantly affect patient care is constant
- The incumbent gives work direction to other department personnel
- Basic Life Support certification.
- Must hold current valid registration as a pharmacist in the State of Illinois
- Attained the professional degree in pharmacy from an ACPE accredited school (B.S. in Pharmacy, or Pharm.D.)
Preferred Qualifications
- 2 years or more of hospital experience as a registered pharmacist
- PGY-1 rotation completed
- Knowledge of contemporary hospital/clinical practice and service and understanding of general hospital operations highly desired.
