The Ingalls Home Care Physical Therapist is contracted or employed through the Agency is responsible for providing physical therapy services in compliance with all TJC and IDPH standards of acceptable practice, and must be in adherence with all agency policies.
Essential Job Functions
- Evaluates patient’s functional status (muscle function, endurance, range of motion, strength, balance, bed mobility, transfers, ambulation, etc.)
- Evaluates home environment for hazards or barriers to safety.
- Identifies equipment needs for patients.
- Assesses patient’s overall potential for rehab. Participates in the development of the total plan of care for the patient.
- Develops goals and treatment plan based on evaluation results, prognosis, patient, family, and/or caregiver goals.
- Communicates plan of care to referring physician and other health care team members.
- Teaches new skills or retrains patients in once familiar mobility activities that have been lost due to illness or injury, in accordance with Agency policy.
- Trains patients in the use of prosthetic devices.
- Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits all records in accordance with Agency policy.
- Identifies patient needs for other home health services and refers as necessary.
- Communicates effectively with team members, staff, other health professionals, patients, family, and/or caregivers to achieve optimum care for patient, facilitate team interaction and positively represent the Agency.
- Attends and contributes to in-services, case conferences, and other meetings as required by Agency policy to ensure coordinated and comprehensive patient plan of car
- Instructs patient’s family/caregiver and other Agency health care personnel in assisting with the patient’s treatment regime when indicated.
- Participates in discharge planning which may include educating patient, family, and/or caregivers, providing a home exercise program, and recommending appropriate follow-up care.
- Commits to one hundred percent (100%) patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patients, families, visitors, physicians, students, and co-workers.
Required Qualifications
- Current state of Illinois physical therapy license
- Current CPR is required by AHA.
- Two years of clinical experience as a physical therapist. Community/home health experience is preferred.
- One year of home health care experience preferred.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits.

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
