Position Title: Clinical Consultant - Onsite (Case Management Operations)
Shift: 8-hour shift, approximately 8 AM-4 PM CT, Monday to Friday
Travel Requirements: Up to 40-60% travel to various facilities
Health Requirements: Flu vaccination required or mask must be worn from November 30 through March 31
Position Summary
The Clinical Consultant in Case Management Operations will play a key role in the implementation and optimization of the CarePort system. This position serves as a clinical subject matter expert focused on enhancing case management technology to support hospital workflows effectively. The consultant will work closely with the Case Management Director and Program Director to identify gaps, document requirements, and ensure the new technology meets operational needs.
Major Responsibilities:
- Assess system standards, processes, and practices to ensure compliance with best practices and industry standards.
- Act as a clinical expert, guiding the implementation team to develop system standards aligned with case management requirements.
- Collaborate with project leadership to ensure seamless system design, development, and implementation.
- Identify and address workflow inefficiencies, proposing solutions for optimized case management processes.
- Document business requirements to support both current and future system needs.
- Partner with IT, Education, and key stakeholders to integrate project plans across the organization.
- Proactively escalate risks and barriers that could impact the project timeline.
- Maintain strong organizational skills and prioritize tasks to ensure efficient resource management.
- Support future technology automation initiatives within the Case Management department.

Education & Experience:
- Bachelor's degree in Nursing (RN) – Required
- Master's degree in Nursing (RN) – Preferred
- 3 years of experience in system applications – Required
- 3 years of experience in Hospital Case Management – Required
- Experience with CarePort and other EMRs (e.g., Meditech, Epic, Cerner) – Required
Licenses, Certifications, & Training:
- Active Registered Nurse license in practicing state
