Position Overview: Intensive Case Managers are passionate individuals, skillful at building rapport, and responsible for serving the most chronically homeless, vulnerable individuals who require high frequency supportive care. Care ranges from connecting clients to mental and medical health treatment, bridge housing care, housing navigation and placement, companionship, grocery shopping and more.
Work Location base-office for every Case Manager is determined by City Net contracts, with Case Manager's typically being dispatched in City Net vehicles throughout the contract defined geographical area.
Job Duties & Responsibilities
- Provide Intensive Case Management Services to individuals experiencing homelessness in a bridge housing setting which includes motel rooms and residential houses.
- Provides mental health and substance abuse assessments, with the support of drug and alcohol counselors and mental health technicians, to meet a client “where they are” and support the client to achieve their defined goals.
- Connects homeless neighbors to appropriate supportive services, including but not limited to, mental health treatment, counseling, and permanent housing.
- Coordinates and accompanies clients for appointments, such as but not limited to: SSI/SSDI approval, court appointments, mental health and medical treatment, vision and dental appointments, etc.
- Works directly as part of multi-disciplinary care teams to ensure clients receive all the help they need. **
- Develops with the client a strategy and plan to bring about long-term housing and stability for the client.
- Refers, enrolls, and provides CalAIM services to qualifying clients, or any other programs that will provide additional services that will support the client with obtaining permanent housing.
- Works directly with a client, in collaboration with our partners and care team, to obtain and maintain permanent housing as quickly as possible.
- Moves frequently between being inside the office, in the field (both indoors and outdoors at local shelters, parks, restaurants, riverbeds, beside railroads, beside freeways, etc.), or at meetings/events to access various people and fulfill the mission of City Net.
- Goes to office for files, equipment, usage et al, and as needed to fulfill job responsibilities.
- Transports clients in City Net vehicle as needed.
- Maintains a clean, safe and unobstructed workspace, in a vehicle or office.
- These duties are not exclusive and with consideration of the job requirements and other skills, the job description can be added to or taken away from at the discretion of the employee's supervisor.
Requirements
- 1 to 4 years of case management experience with City Net's target populations, including but not limited to; people affected by homelessness, substance use disorders, chronic health and mental health concerns, domestic and intimate partner violence, and/or human trafficking.
- A passion for ending homeless with a desire to make a difference in their lives.
- Must be able to work flexible hours, including evenings and weekends when needed.
- This position involves variable scheduling including, but not limited to, early mornings, late evenings, and weekends. For example, there may be an early morning shift once per week, or a weekly evening outreach.
- For insurance purposes, needs to be at least 25 years old, must have a valid drivers' license, and be approved to drive a City Net vehicle by our insurance company. Employee's driving their own personal vehicle for work purposes will also need to show valid liability insurance.
- Consistently works out of the office and outdoors - regardless of the weather condition.
- Willing to walk/traverse all types of terrain, including rough and outdoor, on a constant basis.
- Needs to be “fast on one's feet” and be able to move quickly in urgent/emergency situations.
- Maintains excellent professional boundaries.

- Effective and professional networking and relationship management skills.
- Demonstrated experience and sensitivity working with a variety of ages/generations.
- Displays patience and self-control with employees, clients, customers, and the public-at-large.

- Working knowledge of the Microsoft Windows OS, Microsoft Office 365, and the Internet.
- Experience gathering and maintaining confidential client records, including intake paperwork, case notes, and client management system data entry a plus.
- Regular, timely attendance.
Nice to have
- Bachelor's degree or higher in Social Work, Human Services, Sociology, Psychology, or related field a desired plus.
- Experience cultivating and maintaining productive, professional relationships with stakeholders.
