POSITION TITLE: Hybrid ICJIA Health Navigator STATUS: Exempt, FT/Temporary GRADE:1 DEPT: Safety and Wellness PROGRAM: Health & Human Services SALARY: $40K-$42K/yr
REPORTS TO: Associate Director of Case Management SUPERVISES: N/A
SUMMARY Hybrid ICJIA Health Navigator (HIHN) is responsible for helping clients and their families navigate and access community services that promote self-sufficiency and well-being. The HIHN offers navigation assistance to a range of services and other resources that improve household health status, quality of life and adopt healthy behaviors.
COLLABORATES WITH: Collaborates with various YWCA Teams, clients and members, volunteers and interns and community partners.
ESSENTIALRESPONSIBILITIES:
Direct Services: Provide health navigation services to survivors of sexual violence and non-offending significant others. Collaborate with clients to develop and implement goal-oriented and integrated plans, based on overall needs and interests. Build, cultivate, and deepen trusted relationships and partnerships with individuals and their families to increase knowledge and reduce hesitancy with managing their health concerns and diagnoses. Promote everyday preventive actions to manage health, provide information to break down myths and rumors, and identify populations at risk. Serves as primary point of contact for survivors of sexual violence seeking accurate information, support with registration/appointments or resources needed to access medical care around hypertension, diabetes and breast/cervical cancers. Identify resources and assist survivors in acquiring basic elements of self-sufficiency while empowering clients to overcome barriers to care, such as generalized fear/mistrust of healthcare system, transportation, food/housing insecurity, immigration status, language, and childcare arrangements. Facilitate clients’ connection with regular primary care provider for ongoing preventive primary care and follow-up. Utilize social media platforms to affect awareness and promote accurate information. Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance. Maintain client confidentiality according to HIPAA, YWCA Confidentiality Standard and other funder regulations.
Member Data Management: Maintain comprehensive member records as required. Maintain all data entry and statistical records for multiple funding sources regarding case management services performed. Maintain member confidentiality as required by law and funding sources.
Professional Development & Case Management Knowledge: Collaborate with counseling and advocacy/participate in team meetings. Participate in regular reflective supervision meetings as required. Complete 40 hours sexual assault training.
Community Engagement: Create, cultivate, and deepen strategic partnerships with public and private organizations to establish valuable and responsive referral connections. Assist in the planning and implementation of special events and/or programs. May be required or invited to participate in community events Assist with planning and implementing special programs e.g. Cervical Cancer/Breast Cancer awareness, Hypertension Awareness and Sexual Assault Awareness Month, "
OTHER DUTIES AS ASSIGNED: Other duties outside of the responsibilities of the position may be assigned, whether for a project, special needs task or other assignment.
QUALIFICATIONS: One year of community or customer service experience is required. A current driver’s license, verifiable good driving record and reliable transportation required. Verifiable connection with a trusted community group, organization or institution. Preference for bilingual/multilingual (English and Spanish) written and spoken. The position requires prior experience with Microsoft Office Suite - Word, Excel, Teams and Outlook. Demonstrated public speaking, writing and presentation skills with well-developed organizational and problem-solving skills. Initiative-taker with strong time management skills
The HIHN must possess the ability to effectively manage shifting priorities and respond effectively to crises and changing circumstances. The HIHN must possess a high tolerance for chaos and high-stress situations, with a service orientation.
*Note: A bilingual Community Health Navigator who works with a foreign language speaking community must demonstrate cultural competency with that community and fluency in both English and the community’s language (oral and written).
EDUCATION REQUIRED: A High School diploma, or GED, is required. Must have previous experience with community work. (I.e. Advocacy, volunteer, etc.). Successful completion of a Community Health Navigator formal training program is preferred.

WORK SCHEDULE: Monday – Friday, 8:30am-4:30pm. This position may require work on some evenings and weekends. Schedule can vary depending on the needs of the agency.
