REQUIRED EXPERIENCE:
- Proven work experience in case management with a preferred minimum of three (3) years experience in a case management position in the healthcare/insurance industry related to Medicaid and/or Medicare
- Clinical and/or dental health experience is preferred
- Professional or personal experience supporting individuals with disabilities, chronic illnesses, and/or experiencing homelessness preferred
- Effective verbal and written communication skills, exceptional interpersonal, customer service, and problem-solving skills
- Ability to build relationships with different types of people, including clients, caregivers, family members, community-based organizations, and members of the health care
JOB DESCRIPTION:
- Receive, manage and resolve referrals for Medi-Cal members' case management services
- Conduct assessments to identify Medi-Cal members' care needs and preferences to develop and adjust comprehensive care plans tailored to their specific needs
- Collaborate with a multi-disciplinary team to schedule, coordinate and monitor complex care plans involving dental and medical providers appointments, receipt of care, transportation, and language services
- Record case information accurately and provide summaries when requested
- Identify challenges related to case management and provide suggestions for improvement
What you should expect in this role
- Hybrid work environment 3 days a week in the office as needed for face-to-face and client meetings with the remainder of the time being remote work.
