The Housing Case Manager for CCAB Family Shelters is responsible for orienting families to the 24-hour program upon entry and for working with families during their stay to attain independent housing for the family unit. Motivating and teaching individuals is crucial to assist families with the transition from shelter living into independent living with confident skills. The Housing Case Manager will work as a member of the Case Management Team with a focus on obtaining stable, independent housing.
ESSENTIAL FUNCTIONS
- Maintain professional boundaries and confidentiality at all times. Review program policies and procedures to ensure compliance.
- Complete all intake procedures upon entry of a family within the guidelines set by Executive Office of Housing and Livable Communities (EOHLC) and program.
- Conduct case management meetings twice a weekor as needed, with the head of the household and all family members 18 years or older.
- Facilitate meetings with collaterals as needed.
- Accompany residents to housing appointments, viewing apartments, and other appointments as necessary.
- Create monthly re-housing service plans ands 60-day assessments in accordance with EOHLC and program guidelines.
- Work with Direct Care employees to organize and prepare resident rooms prior to move-in date.
- Inform Efforts to Outcomes (ETO/ASSIT) administrator of family’s exits and entries. Report and addressdata and other required information into the ETO system as needed.
- Utilize the ETO system for case notes and maintain up-to-date documentation and relevant family information in the database.
- Work is performed in a building that houses homeless families. Daily interaction with staff, the general public, community and families. Use of computers, phones and office equipment.
- Attendance at internal and external meetings and trainings.
- Local travel as needed.
- Other responsibilities as assigned.
QUALIFICATIONS
- Bachelor's degree with a concentration in human service or related area.
- A minimum of 2 years prior experience working with clients in a residential program. Ora recent successful internship with another residential program.
- Bilingual-preferred: Spanish or Haitian Creole a plus.
- Experience working with a homeless population preferred.
- Good listening skills with proven de-escalation skills.
- Ability to work independently and as a team player to accomplish program goals and assist clients.
- Strong documentation and organizational skills.
- Ability to work additional hours to help cover shifts during weather emergencies and staff shortage. Including evening and weekend hours.
- Proficient typing skills and competent with computer use, Microsoft Office Suite. Good internet skills for researching program related information. Interest and ability to use online database for record keeping and reports.
- Keen attention to detail with excellent data collection and writing skills.
- Must be able to lift, pull and/or push 25 pounds with and without supportive device.
- A valid driver’s license with a safe driving record and the ability to drive the agency van as needed.
