As a Hospital Liaison, you will:

• Collaborate with case management teams to support high-needs CKD and ESKD patients.
• Engage with hospitalized members, educate them on program benefits, and facilitate safe discharges.
• Address social determinants of health and coordinate post-discharge support.

Qualifications include experience with Medicare/Medicaid, discharge planning, and EMR systems. Strong communication and organizational skills are essential. Proof of COVID-19 vaccination is required.
