Hospital Liaison - Admission Support Liaison
QUALIFICATIONS:
Health - Ability to meet performance requirements
Education and Experience: 1. Bachelors Degree in SW preferred but not required 2. Prior case management experience and/or Hospital Liaison experience preferred. Skilled Nursing Facility (SNF) experience is preferred. 3. Marketing background is a plus 4. Seeks opportunities for personal and professional growth.
Job Knowledge and Capabilities: 1. Is responsible for maintaining clinical competency as evidenced by application of integrated nursing knowledge and skills, leadership and communication skills.
2. Utilizes the nursing process in assessment, planning and implementing care.
3. Exhibits organizational ability related to workflow, prioritizing to meet the patient care needs
4. Integrates current standards of practice as well as local, state, and federal regulations related to nursing services in the care of patients.
5. Practices continuous quality improvement thinking and problem-solving skills.
PERFORMANCE REQUIREMENTS: Physical Demands:
1. Able to be on feet standing or walking 8-12 hours per day
2. Able to lift 60-70 lbs. on occasional basis
3. Able to bend, stoop, squat and twist numerous times a day
4. Able to see and hear adequately in order to respond to auditory and visual requests
5. Able to speak in clear, concise voice in order to communicate with patients who may be hearing impaired
6. Able to carry out fine motor skills and manual dexterity
Specific Duties and Responsibilities Required:
1. Working with Hospital Case Managers and Discharge Planners to facilitate the smooth transition of patients from acute care to NHC's continuum of care.
2. To support NHC in its efforts to be the provider of choice for acute care hospitals, physicians, payors and the community.
3. Maintains open and ongoing communication with patients and families, providing opportunity and encouragement to participate in decision making.
4. Review and assessment of patient records for appropriateness of placement to NHC centers. Have clinical skills and knowledge of services NHC centers provide to make quick placement decisions.
5. Communicate and collaborate with center Admissions Coordinators / Social Workers about referrals, obtaining medical records, arranging transportation and being a representative of NHC facilities to our Hospital providers.
6. Have knowledge of and assist in the marketing of NHC centers, homecare and hospice.
7. Have knowledge of Medicare, Medicaid and other insurance benefits to be able to answer basic questions
8. Assist in obtaining consent to treat and/or admission agreements to facilitate transitions to NHC centers.
9. Work with a team of liaisons to help serve the 4 NHC centers in the Chattanooga, Tn market with willingness to be flexible in hospital assignments to support other liaisons and centers as we serve our customers: Patients, Hospitals, Case Managers, Physicians, Payors and NHC Centers.
10. Assist with admitting patients as required.
11. Assures the personal dignity and physical safety of each patient. Assist patients to attain the highest level of self-care possible.
12. Attend in-service education programs and participates in RELIAS Training, as assigned, to learn new procedures, develop skills, etc.
13. While this role is typically NHC center based, individual must lead by example, representing NHC values when at respective hospitals, at meetings in their role and in the community.
14. Perform other duties as assigned
Other Demands:
1. Patience and tact in dealing with patients and family members.
2. Review and practice the promises of NHC's Better Way expectations.
3. Willingness to perform a variety of simple repetitive tasks.
4. Have good typing and computer skills to use email, Microsoft Word, Microsoft Excel and other computer programs / platforms NHC relies on for the services we provide.
5. Able to establish and maintain relationships with patients, family members, and other center personnel, while assuring confidentiality of patient information.
