About the Job
Job Description:
The Hospice Triage Nurse accepts referrals, telephone orders and lab reports. She/he triages calls from patients and caregivers and is able to calmly talk them through medication administration or emotional support needs in time of crisis. She/he functions to coordinate the transmittal of information between the Hospice/Palliative Care Director, hospice staff in the field, and physician"s offices. Relays appropriate changes in orders and scheduling to appropriate personnel. She/he is available to support additional nursing functions, which occur in the office and offer direct support to the Hospice Director.
Skills/Qualifications:
Current State of New Hampshire RN or LPN License
Hospice experience, two years minimum Preferred but not required
Requirements:
Completes work and documentation with accuracy
Good written and verbal communication skills
Excellent organizational skills
Ability to manage multiple tasks simultaneously, accurately, in a timely manner, and have a strong attention to detail
