Summary: Social Worker will report to the Director of Clinical Services or the Director of Patient Services. The primary responsibility of the Hospice Social Worker is to provide assessment, intervention, education and counseling to Hospice patients, family and/or caregivers. Hospice Social Worker is responsible to ensure that all documentation and requirements of the standards of participation are completed and documented in the EMR.
Schedule and Wage:
- $24- $30 Hourly
- approx 20hrs weekly
Responsibilities:
- Assesses any critical needs that have been identified and need immediate attention through morning team huddle.
- Schedules daily visits
- Conducts initial Psychosocial and Bereavement assessment within 5 days of admission to the Hospice program and communicates findings to appropriate members of the interdisciplinary team as necessary.
- Counsels' patients and family/caregivers on environmental assessment concerns and develops individualized care plans based on assessment.
- Assists patients and families in determining needs related to financial, legal, and general healthcare questions including what to expect in the death and dying process.
- Assess any needs, traditions and/or rituals related to the patient/family culture/beliefs and environment. Communicates needs to the interdisciplinary team.
- Participates in interdisciplinary team meetings and provides routine updates related to patients' plans of care.
- Completes all clinical documentation in the Electronic Medical record per documentation policy and communicates to interdisciplinary team as needed.
- Reviews and or educates patients and family on advanced care directives to ensure that patients' wishes are documented.
- Participates in planning and referral, as necessary, for patients practical and environmental needs.
- Provides education/information and assists patients and families with accessing community resources.
- Collaborates with "care team" to ensure adequate support and planning for patients care and needs and or concerns are being met.
Qualifications:
- Master's degree in Social Work (MSW)
- Hospice experience preferred.
- 2 years' experience in a healthcare setting with an understanding of the hospice philosophy and/or knowledge of death/dying.
- Have and maintain license to practice as a social worker in the state of Minnesota according to Minnesota statute 418.114(b)(3).
- Must have and maintain CPR certification.
