What's your day like?
- Works closely with the Hospice Department with regards to completion of hospice department clerical and clinical tasks.
- Supports Hospice Team as requested for day-to-day clerical/clinical support and special projects.
- Assists with phone calls, faxing, scanning, copying, and/or emailing of clinical information to assist with management of new patient referrals, transfers, discharges or changes of location or level of care.
- Assists with ordering of DME as needed and coordinates delivery with DME provider and patient/family to ensure smooth process as requested.
- Contacts field staff when health information/triage calls are received that require immediate staff attention, via phone or secure text as appropriate.
- Assists with hospice triage, coordination of referrals and communicates timely with hospice team to meet patient needs.
- Supports staff with visits schedule changes as needed.
- Makes changes to hospice team schedules as requested. Notifies staff and patients/families of changes. Assignments are made with consideration of patient locations, care needs and caseloads.
- Assigns admissions and initial visits within workflow with accuracy and attention to territory and location.
- Responsible for running hospice huddle. Keeps huddle focused and brief.
- Completes scheduling workflows timely and accurately.
- Runs and manages all scheduling reports daily to perform job.
- Notifies Hospice Manager of scheduling conflicts.
- Responsible for organization and management of donated hospice supplies
- Sends narcotic disposal to any patients that are recently deceased.
- Performs hospice visits as directed by hospice manager in accordance with the nursing job description
- All other duties as assigned.

Knowledge and skills required for the position are:
- LPN License
- Hospice Experience and Technology Skills are preferred
