Weekend Hospice (RN) Case Manager - Part-Time

A Part-Time Weekend Hospice (RN) Case Manager is needed in Atlanta, GA.

Titan Placement Group is currently seeking a compassionate and experienced Weekend Hospice RN Case Manager to join a reputable healthcare team in Atlanta, GA. Atlanta is a vibrant city known for its diverse culture, thriving economy, and dynamic healthcare landscape.

Salary and Benefits:

• Competitive hourly pay: $41 to $43 per hour
• Schedule: Every other weekend, Saturday and Sunday (10-hour shifts)
• Mileage Reimbursement: $0.50 per mile
• Cellphone Allowance: $60 per month

Responsibilities:

• Work every other weekend: Saturday and Sunday (10-hour shifts)
• Provide hospice care services to patients in their homes
• Manage a case load of hospice patients, ensuring quality end-of-life care
• Collaborate with an interdisciplinary healthcare team
• Conduct thorough patient assessments and develop care plans accordingly
• Maintain detailed patient records using the provided EMR system

Requirements:

• Clear and active RN license in the state of Georgia
• Preferred experience in Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care, or MedSurg
• Compassionate and dedicated approach to patient care
• Strong communication and organizational skills
