This is a weekend Baylor Position

JOB RESPONSIBILITIES:
Assessment/Planning:
Weekend coordination of services including: Coordination of referrals, prn visits, hospice pronouncements and coordination of visits, etc.
Has a good working relationship with referral sources and performs hospice consults/evaluations for hospice or GIP at referral source sites per request.
Makes home visits that are scheduled by the scheduler, but weekend coordinator is responsible to make changes to the schedule and PRN visits as needed.
Supplies and durable medical equipment (DME) are ordered correctly, as needed and appropriately based on hospice criteria and coverage guidelines.
RN accurately performs patient assessments and adjusts care plan based on assessment.
Relevant data and interventions are documented in appropriate areas of chart (e.g., lab work, vitals, treatments, and education, etc.)
Performs hospice admissions following hospice eligibility regulations
Consults with Hospice Medical Provider and hospice pharmacy as needed on each admission to review medication profile, coverage, etc.
Communicates effectively with hospice team and other health care professionals
Appropriately utilizes resources with the consideration of cost and utilization standards.
Appropriate referrals are placed for services within the hospice team.
RN assesses and educates patient/caregiver on Advance Directives and End of Life of Planning/care needs.
Involves patient/representative in hospice plan of care and interdisciplinary team.
RN is able to complete Admissions with accuracy
RN completes Hospice Information Set (HIS) outcome assessment measures (pain, bowels, and dyspnea) with accuracy.
Aware of the Quality Improvement Performance Improvement (QAPI) plan and process for quality improvement projects for both the agency and Hospice team
Completes required Federal and State forms per regulations at appropriate time points.
Triages all calls while on call and documents those calls in clinical record.
Updates weekend schedule as changes in patient status occur.
Documents all patient calls received in clinical notes of patient's chart.
Provides supervision/support to weekend staff as needed.
Works with Home Care Weekend Intake Nurse to coordinate hospice referrals to meet the needs of the hospice patient.
Completes workflows in EMR timely and with accuracy when needed.
Engages in performance self-appraisal on a regular basis, identifying areas of strength as well as areas for professional development.
Clinician follows agency clinical policies and procedures.
Completes/attends assigned in-services, State requirements and other trainings/competencies for position
Maintains and protects client and agency confidentiality within legal and regulatory parameters.
Adheres to Hospice Nurses Job Description and standards.
Fulfills additional responsibilities as assigned and as necessary, accurately and in a timely fashion.
