Hospice Registered Nurse (RN) RETENTION BONUS
Dependable Hospice & Palliative Care is seeking a compassionate and hardworking Hospice Registered Nurse to join our team. As a Hospice RN, you will provide specialized care and support to patients and their families during end-of-life journeys. You will collaborate with interdisciplinary teams to create and implement individualized care plans for patients in a comfortable and supportive environment. RESPONSIBILITIES:
- Provide expert care and support to hospice patients and their families in consultation with interdisciplinary teams
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient progress, and make any necessary adjustments to care plans
- Educate patients and families on how to cope with end-of-life issues and facilitate discussions about advance directives
- Evaluate and manage symptoms such as pain, anxiety, nausea, and shortness of breath
- Collaborate with social workers, chaplains, and other team members to create safe and effective care plans
- Provide emotional support to patients and families as they navigate the end-of-life process
- Maintain up-to-date knowledge of hospice care regulations and best practices
- Participate in interdisciplinary team meetings and communicate effectively with team members
- Complete documentation and billing in a timely and accurate manner
- Participate in on-call rotation as necessary
JOB REQUIREMENTS:
- Valid and current RN license
- Minimum of 1-year nursing experience in hospice or palliative care preferred
- Knowledge of hospice care regulations and best practices
- Strong assessment and critical thinking skills
- Ability to work effectively in a team-based environment
- Excellent communication and interpersonal skills
- Ability to provide compassionate and supportive care to patients and families
- Willingness to participate in on-call rotation
- Ability to complete documentation and billing in a timely and accurate manner
EDUCATION:
- Associate's or Bachelor's degree in nursing from an accredited institution
- Current RN license
