As a Hospice Registered Nurse Case Managers (RN), here’s a big-picture view of what you’ll do:
- Provide home-based care to patients with six months or less to live, focused on quality of life
- Mange plan of care in a manor that is patient and customer service centered that meets external and internal needs including prompt and proper documentation
- Work with a team of physicians and oversee LVNs, CNAs and aides providing patient care
- Teach, supervise, and counsel the patient and family members regarding the nursing care needs and other related problems
- Take a holistic approach to care that engages the patient’s mind, body, spirit and family
A few must-haves for Hospice Registered Nurses Case Managers (RNs):
- Graduate of an accredited nursing program and active RN license in California
- Minimum of 1 year of nursing experience in hospice
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills and compassion
