Hospice Referral Coordinator - Tandym Group

A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff in the Bronx as a Hospice Referral Coordinator.

Responsibilities:

The Hospice Referral Coordinator (RN) will be responsible for:

• Evaluating and establishing a plan of care for patients referred for home health services
• Facilitating admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members
• Developing and maintaining positive relationships with Physicians, Social Workers, Case Managers and Discharge Planners
• Performing other duties, as needed

Qualifications:

• 2+ years of experience in Home Care Nursing
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• Proficiency in Physical Assessment and Interviewing skills
• Solid assessment, clinical, and documentation
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

• Bachelor's Degree in Nursing
