Responsibilities:

Deliver direct patient care and RN Case Management services.

Conduct assessments and monitor changes in patients' health.

Collaborate with doctors to manage medications and vital signs.

Requirements:

Illinois RN license and at least 1 year of hospice field experience.

Compensation:
