Shifts Schedule:
- 3:30 pm to 12 am (Midnight)
Position Summary of a Licensed Vocational Nurse (LVN):
- Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
- Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
Minimum Qualifications:
- Current CA Licensed Vocational Nurse (LVN) license
- Current IV Certification
- Current CPR card for Healthcare Providers
- 1 year of Licensed Vocational Nurse (LVN) experience
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Demonstrates excellent verbal, written communication, and organization skills.
