Hospice Liaison RN, Weekend Lead

Full Time, Tuesday through Saturday

Stony Brook, NY

The Hospice Liaison, Weekend Leadis responsible for assessing, planning and developing referrals, establishing and maintaining professional relationships with key physician groups and referral sources.  This position is also responsible for coordinating patient transfers to our two Hospice Inpatient Centers.  This position will work closely with our Business Development Manager and assume leadership responsibilities in their absence.

The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times.

Minimum Requirements:

• Current and valid NY State license to practice as a Registered Nurse
• Bachelor’s degree in Nursing
• Minimum of three years’ acute care nursing experience, preferably in Hospice, Palliative Care and/or Oncology
• Strong computer skills, including Electronic Medical Records. EPIC a plus
