Details:

• Build relationships with referral sources
• Coordinate patient care with families and clinical staff
• Educate new patients on admissions
• Process DME orders
• Track referrals and perform clerical duties
• Ensure HIPAA compliance

Qualifications:

• 2+ years of experience in hospice or healthcare preferred
• Proficient in Microsoft Word and Excel
• Strong organizational and time management skills
• Excellent customer service and telephone skills
• Knowledge of medical terminology
