Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic's hospice team , includ ing physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers , care for patients where they call home (home, nursing facilities, assisted living facilities, etc.).
A caregiver in this position works 8:00AM-4:30PM. The territory this position will cover may include the following and surrounding areas: Sheffield, Avon, Bay Village, North Ridgeville and Olmsted Falls.

A caregiver who excels in this role will:
- Perform basic patient care activities.
- Assist with procedures ordered by physician and supervised by an RN, PT, OT or ST as appropriate .
- Assist patients with tending to personal care, activities of daily living and transfers/transport.
- Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
- Assist with keeping the patient's environment clean and orderly.
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
- Escalate change in patient condition to patient's physician as appropriate .
Minimum qualifications for the ideal future caregiver include:
- High school diploma or GED.
- Certification and registered with the State of Ohio Nurse's Aide Registry (STNA). Please include your STNA number on your application.
- One year of clinical experience as an aide in an acute, rehab or skilled nursing setting.
- Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred qualifications for the ideal future caregiver include:
- Basic Life Support (BLS) certification through the American Heart Association (AHA). This will be required within the orientation period prior to providing independent patient care.
- Hospice and Palliative Care Nurse Assistant (CHPNA) certification.
- Home care experience.

Physical Requirements:
- Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
- For field staff only: Able to drive to patients' homes and/or facilities.
- Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
- Physical Demand requirements are in excess of those for Light Work.
- Follows standard precautions using personal protective equipment as required.
