What you will do:
The Hospice Clinical Manager will direct and specialize in hospice care for hospice patients in their place of residence. The Hospice Clinical Manager is responsible for assisting in the overall coordination and supervision of the plan of care for Hospice patients and caregivers, including home, nursing home, assisted living and inpatient care. The Hospice Clinical Manager coordinates with the Hospice IDG team, patient, caregivers, Hospice Medical Director, Primary Care Physician, Specialist or other providers to work towards plan of care goals. The Hospice Clinical Manager assumes responsibility for coordination of care, serving as a liaison between the Hospice IDG team, patients, caregivers, pharmacies, facility staff, physicians and other providers. The Hospice Clinical Manager oversees and monitors the continuum of care for all patient care based on provider orders, compliance, departmental policies and procedures, state and federal laws and regulations, accrediting agency best practice. The Hospice Clinical Manager actively attends, participates and leads meetings as appropriate. The Hospice Clinical Manager performs supervisory visits. The Hospice Clinical Manager performs on-call responsibilities.

Required Education:
Bachelor’s Degree in Nursing
Preferred Education:
Master’s Degree in Nursing
Required Skills:
Nursing skills as defined as generally accepted standards of practice. Effective, professional communication skills. Ability to multi-task. Ability to utilize Microsoft Office effectively.
Preferred Skills:
Experience with Home Care Home Base, Meditech
Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; CPR certified; current Texas Driver’s license; current auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
Hospice and Palliative Care certified; Wound Care certified
Required Experience:
Three years’ experience in respective discipline, experience in supervisory position
Preferred Experience:
Hospice or Home Care experience
