Statement of Position/Position Summary
This position will assume overall responsibility for the implementation, delivery, coordination and supervision of hospice services provided in the homecare and facility settings.
Qualifications and Education requirements
Graduate of an accredited school of nursing: Bachelor’s degree in nursing required, Masters Degree preferred.
Registered nurse with current licensure to practice professional nursing in the state of MI.
Minimum of 3 (3) years of clinical experience including 1 (1) years in a managerial role in a healthcare environment; preferably a hospice environment.
Demonstrated skills in administrative performance, including expertise in written/oral communication, supervision of staff, documentation requirements, service coordination and case management.
Comprehensive knowledge of nursing practice, applicable rules and regulations and industry standards pertaining to hospice care.
Computer proficiency and a working knowledge of Home Care Home Base HCHB and Microsoft programs required.
Current CPR certification.
Valid MI Drivers License and daily access to a reliable and insured vehicle.
Belief in the hospice concept.
Position Role and Responsibilities
Serve as a member of the leadership team for this Agency.
Assume overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and facility settings.
Assist in the development of agency goals. Develops, recommends, and administers Agency policies and procedures.
Supervise all clinical staff including RN, LPN, Hospice Aide, Social Worker, Chaplain, Educator, Massage Therapist, Music Therapist and Medical Office Assistant.
Provide information for inclusion in written performance evaluations for all direct report staff and submits written performance evaluations in accordance with policy requirements.
In conjunction with the Educator role, identifies the need for staff education and assists in the development of in-service programs for members of the IDT team.
Provides leadership and counsel to clinical personnel to enhance staff development.
Participate and provide leadership during IDG meetings and other meetings/committees related to clinical practice.
Participate in events of Amara Hospice as a member of the leadership team.
Conduct supervisory visits to allow for observation of clinical performance of assigned team members.
Supervise the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations and legal and ethical standards.
Assist with oversight and responsibility for the Quality Assessment and Performance Improvement (QAPI) program, to include:
Development of annual QAPI plan
