Hospice Case Manager – Weekend On Call

We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Gwinnett County area of Georgia. They are a family-owned organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.

Our client is offering the following benefits:

• Annual Salary: $80,000-$90,000
• Health Insurance-Employer Pays % COBRA
• Dental And Vision Insurance – Employer Pays % COBRA
• Mileage Reimbursement-$0.48 Per Mile
• PTO-14 Days
• Paid Holidays- 7 Federal

Responsibilities:

• Hours: Friday 5pm – Monday 8am Every Weekend
• Take Incoming Phone Calls – Go Out in Field When Necessary
• On Call for Dekalb, Fulton, Gwinnett, Rockdale, Henry, Paulding, Clayton, and Fayette Counties

Requirements:

• Registered Nurse in the State of Georgia
• 2-3 year of Hospice or Palliative Care Experience-Preferred
• 2 years of in-Home Patient Care Experience-Required
• On Call Experience – Preferred
• Familiarity with EMR System MatrixCare-Preferred
