Role
Based on the vision of this role within the context of the 4Sisters’ full-service case management model the Hope & Healing Facilitator is a field-based position responsible for supervised parent-child and sibling visitation, placement stability, and child well-being as prescribed in applicable Florida Statute, Florida Administrative Code, and CFOP to ensure that safety, permanency, and well-being goals are met.
Essential Job Functions
- Work in partnership with children, families, caregivers, and other stakeholders to achieve permanency, safety, and well-being.
- Facilitate and/or coordinate parent-child and sibling visitations as required and/or requested.
- Engage caregivers through clear communication regarding role and process.
- Co-facilitate TBRI® Caregiver Groups.
- Coordinate and communicate regularly with Permanency Primers© and Case Management team about supervised parent-child and sibling visitation, placement stability, and child well-being as prescribed in applicable Florida Statute, Florida Administrative Code, and CFOP.
- Meet with supervisor and be prepared for scheduled supervisions.
- Attend, participate, and be prepared for case staffings and huddles.
- Attend all required trainings.
- Participate and learn information presented during trainings to gain proficiency in all areas of responsibility.
- Use tools provided to maximize the effectiveness of the Case Manager role.
- Follow agency policy and procedure.
- Enter all case-related activity into FSFN within 48 hours in the required format.
- Advocate and innovate for better ways of doing business.
- Keep up to date with Hope & Healing visitation model through supervision and training.
- Review FSFN to understand reason for removal, conditions for return, visitation status, next court hearing, assessments and other information to help you best prepare for contact with caregiver and visitation.
- Use TBRI® Principles and Strategies in building relationships with parents and caregivers.
- Coordinate child well-being activities timely as required.
- Encourage caregivers throughout week using resources provided by 4 Sisters Solutions.
- Engage caregiver in problem-solving and access resources provided as needed.
- Document contact and attempted contacts in FSFN/note capturing system within 48 hours of activity.
- Communicate need for resources to organization timely.
- Monitor and respond to communications with 4 Sisters Solutions throughout the week.
- Demonstrate team cohesiveness with the primary Case Manager and Permanency Primer© through aligned communication regarding permanency and case status.
Required Education/Training/Certifications
- Bachelor’s degree in social work (preferred)or related area of study or a master’s degree in social work or a related area of study from an accredited college or university. Related areas of study include bachelor’s or master’s degrees in human services, criminology, juvenile justice, psychology, sociology, counseling, special education, education, human development, child development, family development, marriage and family therapy, and nursing.
- Must pass or receive exemption from if allowable by contract background screening process to include local law enforcement, Level 2 criminal background check, Diana Screen®, have a valid driver’s license, and pass an MVR system background check.
- Must hold a Child Welfare Certified Professional (CWCP) credential and uphold the FCB Code of Ethical and Professional Conduct
- Experience: 2 years post-full CWCP credential. It is expected that as the Case Manager is advancing through the Tiers that their proficiency/mastery of their role is advancing as well. Advancement to the next Tier cannot occur if employee is under a corrective action. This must be satisfied prior to advancement.

- Adaptive technical skills to navigate software that might be unfamiliar or used infrequently such as Teams, SharePoint, etc. as well as FSFN.
