PURPOSE AND SCOPE:
As a Home Therapy RN, you will provide and coordinate clinically competent care for an assigned group of patients, ensuring comprehensive assessment, planning, intervention, and evaluation. You'll assess and manage patients' responses to home dialysis training and treatment, adhering to protocols, and communicate patient-related issues to physicians as necessary. This role involves participation in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and operations within the End Stage Kidney Disease (ESKD) healthcare team.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Perform all duties in compliance with Client Kidney Care policies, nursing standards, and regulatory guidelines.
- Collaborate with Supervisors, Educators, and other RNs to provide ongoing, systematic collection and analysis of dialysis data.
- Document patient assessments and responses to treatment; adjust plans as needed.
- Educate patients and families on dialysis and treatment options, assessing their learning needs.
- Direct and provide comprehensive patient care alongside home care partners and ancillary staff.
- Administer medications per prescriptions and document effectiveness.
- Initiate emergency response measures as required.
- Participate in staff training and onboarding for new employees.
- Ensure accurate lab collection, processing, and scheduling of missed collections.
- Document expected outcomes and collaborate with the interdisciplinary team to update care plans.
- Assist in identifying and training Home Dialysis candidates and Home Partners.
- Assess the home environment for dialysis candidates, addressing barriers to care.
- Conduct formal training for Home Dialysis patients and their partners on equipment operation and maintenance.
- Perform additional duties as assigned by the Supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
- This role involves direct patient care, requiring heavy lifting, moving of equipment, and assistance with ambulation.
- Prolonged standing and bending are common; may require moving equipment and lifting up to 30 lbs.
- Work in a healthcare environment with temperature control and moderate noise levels; potential exposure to infectious materials.
- Participation in on-call rotations and travel to various sites may be necessary.
- Daily tasks include desk work, computer use, and patient interactions.
SUPERVISION:
- Oversee LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge after meeting specified qualifications.
EDUCATION and LICENSURE:
- Graduate from an accredited School of Nursing.
- Current state licensure and CPR BLS Certification.
- Must meet practice requirements in the state of employment.
EXPERIENCE AND REQUIRED SKILLS:
- Minimum of 1 year of nephrology nursing experience as an RN within the last two years.
- At least 1 year of experience as a Home Dialysis Therapies Registered Nurse.
