About The Position
Job Title: Hospice Aide/Home Health Aide Job Specialty: Rehabilitation Therapy - Hospital Aide, Home Health RN Job Duration: 13 weeks Shift: Days, 8:30 am - 5:00 pm (Schedule can be flexible based on patient’s availability) Guaranteed Hours: 40 hours per week Experience: Minimum of 1 year License: Current California driver's license Certifications: Current California Home Health Aide Certification, Current CPR certification - American Heart Association BLS Health Care Provider Must-Have: Automobile insurance with the minimum Bodily Injury Liability Insurance coverage per agency policy Job Description: - Provide personal care, home hygiene services, and other supportive services to patients receiving hospice care at home. - Assist patients with bathing, grooming, and toileting activities. - Monitor vital signs and assist with feeding patients, as specified by the case manager or supervisor. - Maintain a safe and clean environment in the patient’s home. - Use assistive devices such as Hoyer lifts, canes, walkers, and wheelchairs. - Offer comfort measures and integrative therapies, including companionship and conversation. - Communicate with the case manager about patient status and participate in care planning. - Assist in training families in caring for terminally ill patients. - Preserve patient dignity and respect spiritual and cultural beliefs. - Perform tasks requested by the case manager to assist patients in obtaining health services. - Utilize electronic devices to access care plans and document visits. - Communicate promptly with patients, families, and department staff regarding scheduled visits. - Attend and participate in interdisciplinary group meetings and training sessions. - Maintain proficiency in providing quality hospice aide care and adhere to agency policies and procedures. - Mentor new clinical providers and assist with their orientation. - Engage in quality improvement activities related to areas of practice.
