Please note that this is a per diem position that is paid on a per visit basis. It is ideal for a ST looking to supplement their income, pick up additional hours and does not want a set shift type of schedule. You will be expected to visit patients in their homes with in Santa Clara, Alameda or San Mateo counties.
InTouch Home Care Inc., is currently seeking a self-motivated Speech Therapist to deliver exceptional care and is enthusiastic about working with adults of all ages. The Home Health ST is responsible for the assessment and evaluation of the patient, and works to achieve better communication and swallowing abilities, and improve their quality of life. This may include difficulties with speech, language, voice, cognition, and swallowing. Based on their assessment and evaluation, the Speech Therapist determines a treatment plan, executes interventions aimed at improving and enhancing the patient's well-being, and monitors the patient's progress effectively.
SUMMARY
A Physical Therapist for Home Health must have:
- Remarkable knowledge of speech therapy principles, theories, methods in their application in a rehabilitation environment.
- Great skills in providing a variety of treatments.
- Profound ability to communicate effectively both written and oral.
- Exceptional ability to understand the role of therapy in the context of the patients age, needs and environment.
- Exceptional ability to prepare and maintain records with accuracy and discretion
- Proven sound clinical reasoning in treating and planning, implementation and monitoring patient progress.
- Ability to recognize when patient protocols or treatment plans need modification to meet appropriate patient needs and report to the therapist.
ESSENTIAL JOB FUNCTIONS
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete assessment and history of current and previous illness(es).
- Regularly re-evaluates patient needs.
- Initiates a care plan and makes necessary revisions as patient status and needs change.
- Care plan establishes measurable goals and outcomes using standardized assessment measures by which to create progress benchmarks. Includes the patient and family in planning process.
- Initiates appropriate preventive and rehabilitative procedures.
- Counsels the patient and family in meeting ST needs
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Completes documentation of visits within 24 to 48 hours of seeing patient to ensure reimbursement.
COMMUNICATION
- Maintains open communication via the agency communication method (HIPAA compliant messaging app) with regard to visits schedule, progress, and any issues that arise through the duration of patient care.
- Communicates with interdisciplinary team, agency staff, patient's primary care physician and their family to coordinate patient's care plan.
- Communicates any changes in patient's condition to the patient's physician and obtains/receives and logs any physician orders as required.
ADDITIONAL DUTIES
- May be expected to supervise any aide staff on a biweekly basis
POSITION QUALIFICATIONS
- SLPA license and registration by the state.
- CPR certification
- Excellent observation, verbal and written communication skills, problem solving skills, PT skills per competency checklist
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Excellent interpersonal skills.
- Home health experience preferred.
