About the Job
We are currently taking applications for a Home Health RN Case Manager. The Home Health RN Case Manager works independently with groups of clients to provide initial and on-going assessment of the client’s skilled needs, implements individual plans of care and make referrals to others disciplines and /or revise the plan as necessary.
Requires Virginia RN license, CPR Certification, and at least one year of nursing experience, preferably in Hospice, Home Health, or a skilled nursing facility. Valid VA driver’s license and proof of insurance required.
Pay range: $38-$42/hr DOE
Background/Drug tests, seasonal flu vaccine required.
