  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
• Valid Regular Driver License (DL)
• Are you okay with the required travel for this position?

Key Responsibilities

• Provide management and delivery of patient care plans
• Collaborate with the care team
• Support patients in Monroeville, PA, and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule
• Utilize tablets, mobile phones, and EMR software

Schedule and Shift Details

• 40 hours per week, day shifts, occasional on-call

Travel

• Occasional local travel

Home Health LPN – Position Details

• City/State: Monroeville, PA
• Territory/Driving Expectation: 20-25 miles surrounding Monroeville, PA
• Top Experiences: Prior home health (HH) or hospice experience preferred, at least one year of med/surg experience preferred, minimum one year of nursing experience required
• Top Misses (experiences we don’t want): Private duty, dialysis, or other limited-scope areas
• Pay Rate/Salary: $27-30 hourly
• Schedule: Monday–Friday, 8 a.m.–5 p.m., one on-call evening (5 p.m.–10 p.m.) per week, and one weekend per month
