POSITION SUMMARY: A licensed Registered Nurse who plans, organizes, and directs home care and hospice services according to physician treatment plan. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The professional nurse may assist with public health programs and serve as RN case managers for hospice client assignments.
ESSENTIAL FUNCTIONS/DETAILED DUTIES:
- Conducts home visits as assigned, assessing the patient's condition, progress toward goals including discharge planning needs, providing patient care, teaching and other skilled interventions as ordered.
- For home health completes an initial home health assessment of patient and family to determine home care needs. For hospice completes initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. For both, provides a complete physical assessment.
- Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by physician.
- Assess and evaluates client/family needs realistically and accurately and documents accordingly in electronic health record. Includes client/family in planning and rendering care, providing emotional support.
- Organizes workload efficiently and remains flexible to change. Anticipates and initiates work responsibilities. Actively seeks additional responsibilities and willingly assists co-workers.
- Prepares clinical notes and updates the primary physician and/or Medical Director (hospice) when necessary.
- Maintain up-to-date plans; new POC ready to be sent within 5 days, recertification's ready to be sent by start date.
- Documents care in the client's clinical record by the end of the second day following visit or within 5 days of admission.
- Regularly re-evaluates patient nursing needs, as part of nursing visit. Communicates with the physician regarding changes in client condition or other patient needs; obtains/receives physicians ordered as required.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Assumes responsibility for supervision and evaluation of Home Health Aide. Supervisions completed timely according to agency policy.
- Participates in on-call duties as defined by agency.
- Actively participates in quality assessment and improvement activities.
- Attends home health care conferences and/or interdisciplinary group meetings as requested.
- May be delegated to serve as RN Coordinator for hospice interdisciplinary team facilitation and case management coordination.
- Assists with Public Health programs as needed.
- Other duties as assigned.
POSITION QUALIFICATIONS:
- Graduate of an accredited school of nursing.
- Licensed as a Registered Nurse in the State of Iowa.
- Maintains a current CPR certification.
- Two years of professional nursing experience desired, but not required.
- Self-directed and able to work with minimal supervision.
- Appearance and personality appropriate to deal with clients, visitors, and related services agency personnel.
- Valid driver's license and proof of motor vehicle insurance.
