Pay: 89K-95K
Schedule:
- Full-Time
- Hours / Days: M-F 8:30-5:00
- Weekends? Only when assigned Admin On call, (holding the phone)
- 40 hours / week
Role Overview:
We are seeking a motivated Director of Nursing with experience in Home Health Care to join our team in Colorado. The ideal candidate will oversee clinical operations, ensuring high-quality patient care, managing and mentoring nursing staff, and driving growth in our patient census. This role offers the opportunity to make a significant impact by building strong community relationships and ensuring compliance with healthcare regulations while promoting optimal patient outcomes.
Key Responsibilities:
Oversee Skilled Nursing Services:
- Direct and manage all skilled nursing visits within the home health care setting, ensuring patient care is delivered in accordance with established standards, regulations, and individualized care plans.
- Supervise and guide nursing staff to provide high-quality, compassionate, and evidence-based care to patients in their homes.
Staff Management and Development:
- Recruit, train, and supervise skilled nursing personnel (RN, LPN & CNA) to ensure high levels of competence, patient satisfaction, and adherence to company policies.
- Conduct regular performance evaluations, mentor staff, and provide ongoing professional development opportunities.
Quality Assurance and Compliance:
- Ensure that all home health care services are compliant with federal, state, and local regulations, including State, CHAP and Medicare guidelines.
- Implement and monitor quality improvement initiatives to ensure that patient outcomes meet or exceed industry standards.
Care Coordination and Patient Advocacy:
- Collaborate with interdisciplinary teams (e.g., physicians, therapists, social workers) to coordinate patient care and ensure optimal treatment outcomes.
- Act as a patient advocate, ensuring patients and their families receive education, support, and timely follow-up care.
Operational Leadership and Budgeting:
- Oversee the daily operations of the skilled nursing program, including scheduling, resource allocation, and budget management.
- Track and manage program performance metrics, including patient satisfaction, outcomes, and operational efficiencies.
Clinical Documentation and Reporting:
- Ensure accurate and timely clinical documentation in patient records, including assessments, care plans, and visit notes.
- Prepare reports and provide feedback to senior management on program performance, compliance, and opportunities for improvement.
Census Growth and Community Engagement:
- Develop and implement strategies to grow the census by building relationships with referral sources, healthcare providers, and community organizations.
- Represent the home health agency in the local community by networking with physicians, hospitals, discharge planners, and other stakeholders to increase visibility and attract new patients.
- Actively participate in local events, healthcare fairs, and outreach programs to establish a positive presence and reputation in the community.
Qualifications:
Education:
- Registered Nurse (RN) with an active, unrestricted nursing license.
Experience:
- Minimum of 5 years of clinical nursing experience, with at least 2 years in home health care or skilled nursing.
- At least 2 years of leadership or management experience in a home health or healthcare setting.
Certifications/Training:
- Current BLS/CPR certification.
- Familiarity with Medicare/Medicaid regulations and home health reimbursement.
- Additional certifications related to home health care are a plus.
Skills:
- Strong leadership, communication, and interpersonal skills.
- Excellent organizational, time management, and problem-solving abilities.
- In-depth knowledge of clinical standards, home health care regulations, and documentation requirements.
- Proficiency in electronic health records (EHR) and other healthcare technology systems.
- Ability to work independently and in a team environment.
- Ability to travel to patient homes as needed.
