The Personal Care Attendant/ Home Health Aide role offers a unique opportunity to help our frail elderly population to continue to lead connected, meaningful lives. Edenbridge PACE is opening the first PACE program servicing the community in Wards 7 and 8 in Washington, DC.
JOB SUMMARY
This position is responsible for providing personal care to assist participants with activities of daily living and supportive care necessary for the preservation of the home environment. Assists in monitoring the health status and providing a safe environment for participants. This position reports to the Home Care Coordinator RN. The work schedule for this position includes days and occasional evenings and weekends.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, toileting, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned or needed.
- Assists in monitoring participant’s health status (vital signs, weight, skin condition, appetite and changes in functional ability).
- Observes participants taking prescribed medications.
- Reports pertinent information to Home Care Coordinator in a timely manner.
- Documents services are provided to participants consistently and accurately via Electronic Health Records (EHR).
- Participates in participant’s care planning through cooperation with multidisciplinary teams.
- Participates in Quality Improvement programs as assigned
- Other duties as requested/assigned professional demeanor at all times.
QUALIFICATIONS
- Must hold Home Health Aid certification in Washington DC
- High School Diploma or GED is required. Equivalent experience in lieu of education will be accepted.
- Must have 1 year of experience with the frail, elderly or disabled population.
- Must be 21 years of age or older.
- Must have good written and verbal communication skills.
- Must have a current Driver’s License and driving record must be in compliance with Company policy at all times. Must maintain current automobile insurance.
- Must submit to and pass a pre-employment and criminal background check.
- Must be free of all communicable diseases and have all immunizations up to date before providing direct care. Vaccination against the seasonal flu and Covid – 19 required.
- Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
- Operates in full compliance with all applicable federal, state, and local fire, health, safety, sanitation, Emergency procedures, and other Standards prescribed in law or regulations.
- Ability to work independently with minimum supervision.
- Knowledge of how to use and maintain in-home equipment (adaptive, Hoyer lift, etc.).
